How does the command code process work operationally? Ex: Sisko-1-5-Destruct, Picard-Epsilon-7-9-3, etc codes need to be secure while also easily deployable.

There is quite a bit of dependency on command codes used by the officers for various overrides or even self-destruct sequences. Clearly an important security measure for Starfleet. Made me curious to think about how that would work practically to maintain security while also being useable. It would have to change frequently, which would make it difficult for memorization. so how would that be managed and integrated into the operational process of Starfleet?
