Anyone ever try to incorporate fatigue with fighting and spell casting?

I have a group that’s getting ready for session 0. They’ve all mentioned they are wanting to play in a gritty, more realistic style of play.
I’ve looked at the DMG’s version, but they are also talking about adding fatigue as free combat, and having faster suffer side effects or a cost for casting. The more powerful the spell the more it takes out of them, kinda thing.
Anyone ever try to do this? How’d it work out? What sort of things did you implement?
