Good ideas for conditions for players that are impactful but still fun?

Players have sanity levels and take sanity damage over time. I have a list of things they can develop condition wise but was hoping for some fresh takes too? Things like third person speak, talk in rhyme only or far more extreme things like hallucinations and general phobias. Thanks in advance!
