Advice on how to stay focused and not autopilot?

If there is one thing that is truly holding me back, it's my patience. I understand that the neutral consists lots of footsies and safe moves and look for openings but when I play, no matter how much I tell myself not to, I just autopilot just because I know what I'm doing works but the moment figures out what I want to do and adapts, I start to crumble. What do you all do to keep your head in the game and keep yourself from autopiloting? How do you keep track of habits of yourself and the opponent?
