What documentation was actually requested at time of vaccine?

Tried signing my wife up for Walmart appt (no appt available yet anyway) based on 2 qualifying health conditions and it said to bring documentation such as a doctor's note.
Have you actually been asked for such or turned away if it was not available at the time of appt?
Likewise my son and I both qualify under B4. Is anyone asking for a note from employer (I'm self-employed and manufacturing, he is restaurant worker)?
