When do I start soaking my shower head in vinegar?

At what point do you start soaking the shower head in some vinegar, even though water is flowing nicely and there doesn’t seem to be much obstruction?
Do I have to do it at least once a year? Or as long as water is flowing reasonably well there is no need to soak it in vinegar?
