Your Help Needed With Nether Map!

Hello everyone. As some of you may know there is a project underway to mark what cities have locked portals, are uninhabited, or are griefed. This information will be displayed on The Nether Roads Map. We need your help to gather this information, so please post any information you may have.
Edit 1: We are updating the map as we go if you notice errors please tell us.
