What define if an attack is a discipline/elemental one?

Is it if the attack deals damage using the element (so the attack is only elemental if if the attacker has the element) or is it if there is the element on the card?

Also outperform make you deals more damage on the corresponding attack, but does it only boost stat check/stat fail/challenge damage or if an attack also has something like "stat check power 50 : gain fire" it will do an extra 5 damage with an outperform power 5?

Does outperform stack? If a card has 2 stat check of the same discipline and you have outperform, does the outperform applies twice?
