I am filling out the Unemployment Insurance form for federal employees. The one they want to fax or mail. I hope I can get some help because it is a bit confusing, I tried calling EDD but they are always busy and never answer.

I am from California.

1. For section 19, when it asks for employer name and address, do we put for the Census job? The address we see on the paystub, SF-50, or SF-8? They are all different names and addresses
2. For section 25b-25g, again I am confused about which address we use, from the paystub, SF-50, or SF-8?
3. For the "Supplemental form for federal employees - attachment B" on the last page (page 12), what do we write for sections 1-5? The only thing I know to write here is the "three-digit Federal Identification". Again I am not sure which address to put here since all three papers (paystub, SF50, and SF8) have different ones.

If anyone can help me fill this out, that would be great! So I can finally submit my application.
