16.5 year old cat - moving

My grandmother just passed away and I’m moving my disabled grandfather in with us. They have a 16.5 year old cat that my grandfather can’t take care of and I have a cat averse dog in my current home.
What is your suggestion with this cat? We took it to the vet and they recommended not putting him down even though we explained the 800 mile move we need to make.
