Return from long term sick leave

I will be potentially returning to work in March after a long term sick leave. My position is in one province and I worked from home, but I worked for an office in the NCR. Iā€™d like to work for a position in my home province, from home/hybrid, but Iā€™m not sure if I can handle the stress of another competition right off the bat etc. Part of my burn out was living/working in different time zones, and feeling disconnected working with an office that all worked together in person for years while I had never met them aside from MS Teams. Is there a way to be accommodated a different position with help from HR for medical reasons? I figure I should start asking these questions now. I appreciate any insight!
