First UTI ever in November, can't get rid of it

Hey! So (27f) had my first ever UTI in the beginning of last November, was prescribed antibiotics after trying to cure it with cranberry capsules and nearly giving myself a kidney infection. Was better after a couple days but the some symptoms never went away: more urgency in peeing, ocasional pain shooting up urethra after peeing, ocasional pain before peeing and a dull pain in my perineum. I've seen several urologists as well as gyno, taken several antibiotics for what was supposedly a very normal UTI (e coli), taken cranberry and dmannose and done everything good but I still can't get rid of those same last symptoms. I've been put on a 6month dose of nitro but I'm honestly afraid it won't work and I'm getting desperate. What can I ask doctors? Please mind that I live in Portugal where I can barely get doctors to pay any attention to my symptoms (i also have hashimoto thyroiditis) Writing because I was hopeful about changing to a new contraceptive pill a couple days ago as it did ease some symptoms but today was a very uncomfortable day and I'm getting depressed
