Here are some troubleshooting steps to try before filling out the form:
~Perform a clean install of graphics drivers
~Ensure your drivers are updated
~Ensure you have the latest version of Windows installed
~ Run a Scan and Repair through BattleNet

I'm running an AMD config with Ryzen and Radeon 5500 XT. Do you Nvidia guys have problems as well or could it be AMD related?
