Needle in chuck primal

My cutter today sent me a picture of a hypodermic needle broken off into a chuck primal. Lucky for us he caught it before a customer did. Is this something we should be looking out for more? I couldn’t imagine a customer finding this in their gums. Have y’all ever found anything of the sorts? Let’s hear some horror stories.

Isn't that usually poultry though? I don't think they inject beef.
