Has anyone forgot using condoms because of the hear of the moment?

How common is it that on the heat of the moment condoms are not used, even on a guy met for the first time, a one time fuck. Also with the heat of the moment, they guy has finished inside? It has happened to me with guys I have met at the bar or swingers club or parties and because of the heat of the moment we forget about the use of protection and they are so in heat the finish in me.
