Depositing via credit card/Withdraw

Has anyone withdrawn after using credit cards for deposits and does Bovada make you fill out a CCVA (credit card verification agreement) before a withdrawal is processed? Apparently they ask for that even for withdrawal via btc. I use btc for deposits now but I have deposited a few times via credit card for special promos in the past. I’m just concerned because I had to get my physical credit card number changed a few months ago so I wonder if there will be issues if they want me to somehow verify the old one. My new credit card is linked to my bank the same, just a different number.
