My hair falls off when I wash it or scratch it

So I've been having this problem for a few years now I'm 26 going on 27 on Sunday I'm mixed (black and white)btw But sometimes when I wash or scratch my hair, it falls off its very small amount How do I fix this or do I go blad or buzzcut My hair routine is to wash it once a week,I rarely brush/ comb it or put anything in my hair cause I wear a hat most of the time P.s sorry for bad formatting on mobile
