What is causing the Game Over (Act 3)?

I've been transformed into a Mind Flayer two separate times now and can't figure out what's going on. First time I fought the Steel Watch at the southern gate, won the fight and it game over'd me. Second time I fought Dribbles after he tried to force me up on the stage and once I won the fight it game over'd. What is going on to trigger this and how do I prevent it?
