I love the idea of "examining" a couple for fertility and breeding purposes. I'd examine each separately and then together.
During each exam, they would need to perform some various exercises for me before finally being asked to try and procreate exactly how the doctor asks. I could see it being very erotic for the couple, which is the big goal.
It's certainly a very deep take on the medical fetish, but one I would love to live out sometime.
