Can someone explain how to awnser "how's it going"

Is 'fine thanks ' enough or do I have to ask about them too?
Does it require eye contact?
Can the discussion stop after the second person tell they are doing good?
Does it always have to be a 'positive mood' awnser?
Any insights on how you experience greeting will be appreciated :)
EDIT: thanks for all your answers. I guess I understand why people tell me I tend to be a little 'rude'. I'll try to get my script together since social life is about to come back
