Had these taps installed...

Had several sets of these taps installed throughout the house, by a plumber. Each of them have two issues:

1) To stop water coming from the spout/faucet/showerhead, you have to turn them off incredibly tight. Is there a way to make them stop the water flow without having to turn them off so hard?

2) Some of them keep coming off because a small bronze screw-in ring behind the handle keeps coming out. (See pictures). I have to keep screwing them back in by hand or fork every few days. Does anyone know how to fix this? Is there a tool to screw them in properly?
