Hi, Milenka is a 13 year old Doberman pinsher, she has never been si k more than some fleas and a tick or 2, her regular weight was 27 kg (59.5 pounds), she is now 25 kg, around 55 pounds 2 weeks ago we notice her rib cage bones were visible and she was skinier than before (she has always been slim)
We talked to our vet and did dome test, the snap test show positive to ehrlichiosis and a light positive to anaplasmosis.
We has some blood test done (dec 18) showing signs of anemia, the blood work showed the following
Result Reference value
Urea 160.3 17.6 - 51.8 mg/dl
BUN 74.91 7-23 mg/dl
Creatine 3.52 0.6- 1.2 mg/dl
BUN creatin 21.28% >20% indicates pre renal Relation cause
At the bottom it said to discard hyperazotemic due to renal failure and to look for another cause for a pre renal issue
The blood for this was taken while she haven't eaten in at least 8 hours, only water
Only the snap and a small blood test is done, no smda or urinary test or anything else
At this point my vet send her 250 mg of doxicilin every 24 hours 20 mg omeprazol every 24 hours (to prevent her for tumy issues by the doxicilin) 20 mg prednisone every 12 hours for 3 days, then reduce to half every 3 days Vitamins for the anemia
He also recomend to give her oral electrolyte, he didn't mentón any specific a mount, and also give her some home made unsalted chicken stock and some chicken with her food, i asked if we should give her anything for the high creatin and BUN, he said no at this time.
At this point my dog is looking good, she is playful and eats her food OK, we fed her dry food with the chicken stock and some chicken, she has acces to clean water all day long, she seems to eat normal and drink the regular amount of water
On dec 23, my mother in law tells me she has been noticing my dog sometimes her back legs kinda wobble and that she has been drinking more water, we get worry something is wrong so we called the vet againg, we do more blood test with the next Result (this is a more complete test)
Result Reference value
Rcb 5.78 5.65-8.87M/uL
Hematocrit 35.2 37.3-61.7%
Hemoglobin 11.2 13.1-20.5g/dL
MCV 60.9 61.6-73.5 fL
MHC 19.4 21.2-25.9 pg
MCHC 31.9 32-37.9 g/dL
Creatine 7 0.5-1.8mg/dL
BUN >130 7-27 mg/dL
Total protein 8.4 5.2-8.2 g/dL
Globulin 5.6 2.5-4.5g/dL
ALT 133 10-125UL
The blood for this test was taken like 1 or 2 hours after she ate some redmeat idk if that could elevate the Creatine and BUN
Now my vet tells me to stop the prednisone, keep everything else the same, he tells me, he has bad news, my dog is in renal failure but he can't give me more info on the severity of such, he doesn't ask for more test and said there is nothing else to do, told me to give my dog Azodyl, and to change the food to only royal canin renal support, since the test came back the 24 of dec late, every thing is close so i can only get the royal canin, that night my dog eats the food with no problem, dec 25, eats the food OK
Dec 26, she doesn't want the royal canin no more so we basically spoon fed her
Mood wise she seems fine, her hair looks really good as always, it shine very good, she is still playfull, if you call her she come fast, if you are eating food (chicken, beef, vegetables, etc) she ask for the food you are eating and if you give her she will eat no questions asked, beside the weight loss she doesn't present any other synthoms, no nose bleading, no swelling of joints, the lack of apetite for her food but at the same time asking for mine is something that idk how to interpretate.
My wife and i are really woried, we don't want her to die, and we don't know if we should trust this guy anymore, when i asked the first time how to lower the urea, creatine and BUN he said not to worry, now 1 week later he tells me my dog is in renal failure but at the same time he doesn't offer any treatment like idk iv water or a blood transfusion to lower the overall levels of toxins, i've been reading on the internet a lot of stuff he didn't tell me, or asked me, idk how much is True but i learn that if you have high creatine is bad and you should try to lower it asap
What should we do?? Should we take a second opinion?
