When a doctor prescribes you less medication than a full package contains, what happens with the leftovers?

In my experience I never had pill meds outside original package, even if they had to open it to take out extra blisters. And there's always original leaflet included. Which makes me wonder what happens to those taken out blisters. Are they used for another customer, even if without original package, or simply discarded? And, if it's the latter, how sustainable is this approach, given that some meds cost a lot.
