Mine is either when I was in a room with a girl but it was near the edge of the home so the ceiling was slanted. I'm 6'2 so I had to crouch in that part of the room. While we were having sex I stood up rapidly to move around and bonked my head on the ceiling and had to stop. My other is that me and my first girlfriend were doing it in her living room and forgot her mom was home and we got caught.
