Do you have to discuss if it's baby making sex or not?

Sorry for the potentially dumb question. But gay guy here, I have known a few people to get pregnant/have babies recently, but I realised, do you discuss before hand whether it's baby making sex or not? Or do you just use/not use condoms or other contraception? Outside of when people are actively trying for a baby and have issues doing so, I just realised I have no idea how that actually works haha
