Is the idea of diagnostic hierarchy not widely accepted? Or was it at one point and now isn't commonly practiced?

I've noticed in a lot of posts, and in my experience, the "go-to" is always a personality disorder diagnosis before a mood disorder (beyond typical presentations of MDD) or psychotic disorder. I thought the recommended practice was to rule out psychotic and mood disorders prior to a personality disorder diagnosis?
