Manual Transmission Seriously clunking

I have a 1995 Chevrolet Silverado K1500 with a 5.7 Liter V8 Engine, and is paired with an NV3500 5 speed manual, Whenever I went to drive it, It does serious clunking when decelerating or shifting gears, U joints are all brand new, Rear Diff was serviced, Transfer case was replaced, The clutch is fairly decent (Dont know when it was replaced), Truck has 178,100 miles (exactly) and the only thing I can think of is the throw-out bearing is worn, any info about these can be appreciated!
