(DWI AND DUI ARE NOT MINOR TRAFFIC VIOLATIONS.) If yes, you must provide a detailed letter of explanation for each offense and provide certified copies of the final court disposition. If the case has not yet been adjudicated in court, you must provide a certified copy of the arrest warrant, incident report or citation complete with a list of charges, indictments and sentencing and plea agreements, if applicable. (Note: You must respond, “yes” if you pleaded and completed probation as a First Offender.)
