What is the origin of the animals associated with the Four Evangelists in European Christian art, and how did their use evolve?

Matthew, Mark, Luke and John are often represented by or with a man, calf, lion, and eagle. My quick search tells me that this practice is both very old and, at least initially, not consistent as to which animal corresponds with which guy.
What is the origin of this tradition, how did it fit in with early theology, and how did it become such a common artistic motif?
