How did the United Duchy of Jülich-Cleves-Berg form, and how did the Duchy of Guelders get included?

I've been reading up on the United Duchy of Jülich-Cleves-Berg, it's an interesting situation, but several things are unclear to me and I hoped you guys could elaborate

Specificly, what lead to the formation of this unified Duchy?

Also, in most sources I read it says it also encompassed territory of modern Gelderland (e.g. the Duchy of Guelders)
How did this territory get added?

Also, of these 4 duchies, which duchy "led" the union?

Any other interesting information would also be welcome, of course
