I've been diagnosed with PTSD for 8 years now, ADHD for two, as well as Substance Abuse Disorder (Alcohol 100 days clean). I have been told that I can ask for reasonable accommodations in reference to the PTSD/ADHD. I am prescribed some pretty heavy medications in order to sleep and then Adderall for ADHD. I am currently working in the private sector for a building supply company. I have already had issues with being late for missing a meeting or two or having slept in past our 0700 start time. I am a field manager so I'm not tied to an office. I have lost my last four jobs due to issues I credited mostly to my alcoholism. I have to work so I am unable to be out of work long enough to maybe get SSI benefits. No one has said anything to be about it yet, but I can't help but feel that I should be proactive about it now.

Should I go ahead and request reasonable accommodations with HR before it becomes a problem. If I do request them, what would/should my requests look like and how could this help/hurt me in my career? I do have a very vague letter from my psychiatrist , and it doesn't ask for any specific accommodations.
