Freaking out about background check. PLEASE HALP

I will try to be as succinct as possible, here is the situation.

The resume that was submitted to HireRight (the company conducting my background check) looks something like this:

Job Title#1 Company D Jan 2013 - Sep 2014

Job Title#2 Company C Jan 2010 - Jan 2013

Job Title #3 Company A Aug 2008 - Jan 2010

6 months after my hiring in Aug 2008 (Job Title #3) Company A went through a merger with Company B and became an entirely different company, Company C. As a result, there was some organizational restructuring and my job title changed to Job Title#2. Then in 2012, Company C was acquired by Company D and again, job title changed. HireRight thinks that these dates and companies correspond to different HIRE and END dates, they do not. They are just an estimate of the closest I can remember to my job title changing. As a result of the confusion, HireRight listed these as 3 MAJOR discrepancies on the report and sent the report to HR of the company I will hopefully be working for.

I have called HireRight 3 different times to clear the confusion, each time unsuccessfully, but the notes of each phone call recorded in the report.

My question is should I be worried about my offer being rescinded? What course of action would be the best to take?

PLEASE HALP

I have received a written offer that I have signed and returned to HR. I have been given a start date of Feb. 16 My education, criminal history, credit history, and drug test all came back clear.
