Multimeter reading 45V throughout entire house

I've been updating outlets and light switches and using a multimeter and receptacle tester along the way - everything I've tested shows me 45V hot to neutral or hot to ground. It seems to be every circuit in the house so is this an issue with the service lines in the box outside of the house? Main connection at my panel in the basement? Is this a power company call or private electrician?

The panel was just updated from a fuses to breakers prior to buying the house (in the last several months).
