Need help seeing if the wiring is correct.

Need help seeing if there is incorrect wiring happening here. Situation: Fan works fine, however from time to time the light will not turn off. I replaced the switch, and followed the original wiring, and it still not working when it wants. Wiring inside the fan is fine, however I'm convinced I may be the fan. There are four cable's coming into the box, 2,3 &4 are white, black, and ground while cable one is red, black, white, and ground. Cable one feed the top terminals of both switches. The whites are combined, grounds combined and jumpers attached to the ground terminal, and the blacks are combined (except cable 1) with jumper running to the bottom terminals.
