Why is private healthcare so expensive in the USA in comparison to Switzerland when both are private?

I am not from either of these countries so excuse me if I got this wrong, just trying to understand how it works.
Googling around, the average price of a doctor visit costs:
- USA: 250USD
- Switzerland 144USD
Also other treatments seem very expensive in the USA in comparisson to switzerland.
Why are the prices so different when both countries have private health care?
