How to talk to endocrinologist first visit?

40/F I was diagnosed with depression when I was 15, hypothyroidism when I was 16, PCOS when I was 18. I cannot tolerate Metformin (regular and ER). I had Gestational Diabetes in 2014. It went away for 3 years, came back again and was put on Pioglitazone. Then it went away again and was taken off Pioglitazone. I have ADHD, anxiety, HBP and all of this seems like a laundry list of diagnoses. I've been to an endocrinologist maybe a decade or more ago. I hate being told my labs are "normal". Then why do I feel like my quality of life is minimal? How can I plead my case to my new endocrinologist and let her know how hard maintaining a healthy weight has been for me my whole life? It's an uphill battle. I'll lose weight when I get the HUGE motivation or scared about living longer. I always regain the weight because I lose motivation. Please help....thank you so very much!
