23M, 5'9", ~250lbs, I use nicotine vapes and smoke weed.

I haven't been to a doctor in a long time and I'm concerned about my health as it seems there may be underlying issues. I don't have any doctors as I haven't been to a family doctor in probably like 6 years and I'm very concerned. Where can I go to get immediate answers to my questions? Would an urgent care take me and help me with my questions? Or am I going to have to take the long route and find a primary care doctor? The three things I'm concerned with are my kidneys, my heart, and potential diabetes. My kidneys and chest have been hurting for a little while, and I have a history of kidney stones. I have a family history of heart disease and diabetes. I am gravely concerned and don't know where to go for help.
