Took a bad fall. At what point (if any) should I see a doctor?

  • Existing medical issues: Ankylosing Spondylitis, two bulged discs in lower back, spina bifida occulta, dysautonomia.

Primary complaint: Pinched nerve (?) in back radiating down right arm, and severe pain when looking up (right at base of skull).

For my entire life, I've had a pretty sensitive vasovagal response. If I'm in enough pain, or if I feel sick, I will pass out. I always know when it's going to happen and can usually get myself somewhere safe and get to the floor before it happens so that I don't fall. But long story short, I have a new job and didn't want to make a fuss, so I tried to power through it. Obviously that didn't work.

I managed to make it to the bathroom before finally passing out. I have no memory of actually entering the bathroom, but I'm guessing I fell pretty hard. When I woke up I was face down on the bathroom floor. I had a small cut on my temple, my glasses were snapped, knees bruised from hitting the ground, palm and fingertips bruised from presumably hitting the door on my way down.

I felt alright once I came to (just a bit bruised and sore), so I took a small break and just went back to work (assuring my manager that I was fine). However, after the initial adrenaline wore off, I realize I might have done more damage than I'd thought.

First, I have a weird pain in my right shoulder (like where my shoulder blade meets my spine) that radiated down my arm to my elbow. It doesn't feel like a pulled muscle, more like my arm is filled with strings being pulled super tight to the point of pain, if that makes sense? It's like a pressure almost, painful and super uncomfortable.

Second, at the very base of my skull, there's a super sharp pressure-y pain that happens any time I lift my head. Even looking straight-on is difficult, and tilting my head up any higher is nearly impossible without a sudden excruciating pain. The pain goes away once I move my head back down, but obviously not being able to raise my head isn't ideal.

I'm assuming it's just inflammation/pulled muscles/etc from the fall, and that it will go away on it's own. But with so many issues with my spine, I'm also worried that I might have messed something important up.

Should I wait to see if it gets better on it's own, or go get it checked out?
