(26M, Canada) Constant heart palpitations and shortness of breath. Mild chest pains. Went to ER, they said it's nothing. It's still happening, and I'm really worried.

Sorry, this is long, but please read it if you can!

Details:

  • Location: Ontario, Canada.
  • Height: 6'0".
  • Weight: about 200 lbs.
  • Race: North Indian.
  • Existing medical issues: ADHD, autism spectrum disorder, generalized anxiety disorder. Gallstones that have yet to be removed (operation keeps getting postponed due to COVID), but they haven't hurt me in months. I had high blood pressure problems in my teens but I get it regularly checked and doctors have never seen fit to prescribe anything.
  • Current medication: Concerta (methylphenidate), 72mg, daily. Have been taking this dose for over a year with no prior problems.
  • Substances: I occasionally (about twice a month) consume cannabis, usually in edible form, sometimes smoking; I drink about 1 unit maximum, around once or twice a week. I haven't consumed anything since these episodes started.
  • Any other relevant information: I have received both doses of the Pfizer COVID-19 vaccine, the second one in mid-June.

Primary complaint:

About 4 days ago, I started having mild chest pains, heart palpitations, and brief spells of dizziness and nausea. Eventually I started becoming a little short of breath as well. All of these symptoms started in episodes of 1-2 hours, and once they went on for 6 hours. Eventually, as of today, the heart palpitations and shortness of breath became basically constant. I can more or less always feel/hear my heart beating pretty strongly right now, which is very unusual for me. So the chest pains and dizziness/nausea still come and go in episodes, but the palpitations and shortness of breath are constant by this point. Even sitting in my chair writing this I can feel my heart beating loudly and fast.

The chest pain feels a little like a vice clamping down on my chest, from my mid-sternum to my clavicle, equally prominent on both sides, but it has never been severe. I would not describe it as anything more than a 3-4 out of 10, even at its worst. Most of the time it's been maybe a 1-2 out of 10. It's similar to the "tightness" feeling I get during anxiety attacks, but constant, even when I'm calm/lying in bed. Similarly, the shortness of breath is never enough to cause me to gasp for breath, I just feel a little out of breath constantly, as if I'd just exercised a lot. It's also about a 3-4 on 10 at most, but it's slowly getting worse. There is seemingly no cough or cold accompanying the shortness of breath. It feels very much like a lump in my throat/upper chest.

All of this seems to be worse when I am in my apartment (which I need to stay inside since I work remotely). It's a studio basement apartment, if that is relevant. I have had one or two problems with dust and mold, but nothing in the past 3 months. Even if I go outside for a walk, once I'm inside, it comes back quite quickly. I don't know if it's tied to being inside or outside specifically, though.

I went to the ER today and they did an ECG, chest x-rays, and blood work. They found nothing and told me everything seemed okay. The only red flag seemed to be my blood pressure, which was a little high (I think it was around 140/90), but they didn't prescribe or say anything. I was feeling a little better when I went to the hospital -- the heart palpitations had calmed down for the first time in a few days, and the chest pain had lessened. However, once I got back home (as I'm typing this), it's immediately gotten worse again -- the palpitations, mild chest pains, and shortness of breath are all back.

I'm really worried, especially as there's a big history of heart disease in my family -- my uncle (64) passed away from a heart attack this January, and he was not close to the first person to have a heart attack under 70. Please let me know if there's anything I can do, or if you suspect anything.
