Conflicting Advice for Posterior Tibial Tendonitis

Age: 29 Sex: M Height: 5'10" Weight: 155 Location: NYC

I've had PTT (aka PTTD) for over a year (18 months). I've seen a couple doctors including a podiatrist, physical therapist, orthopedic surgeon, and a sports medicine practitioner.

I've had a couple MRIs, and the most recent one showed that the PTT is gone, but it doesn't feel that way. I also have constant dull pain in the lateral dorsal part of the ankle.

The PT and podiatrist say surgery should not be done. The surgeon says it should. The sports medicine guy wants to give me PRP injections, but from my research I haven't seen much evidence that it's effective, and it's not covered by insurance.

I'm getting totally conflicting advice from all these doctors as to surgery vs no surgery.

2 questions: 1) a) If I'm still having symptoms, is it actually possible the PTT is gone? b) The podiatrist said it's CRPS, but I don't think that's correct, and a second opinion agreed. 2) Is there a practitioner I can go to a "tie-breaker" that won't just recommend whatever their speciality is? Like, if I see a surgeon, they always say surgery (I've seen 2), the sports medicine guy says PRP, and the podiatrists always say insoles. I've been doing PT for so long.
