I mistakenly falsified my file

I'm 22f, 150 pounds, quit smoking a year ago non drinker no drugs. For context, my friend had died from a prescription pill overdose. She had a surgery and daved her pills for weeks and weeks and tried to get high but died. That week I already had a doctor's appointment and told them I wanted my file to say I had an issue with pills so I would never receive a prescription. I now realize this is the worst way to have gone about grieving this loss and negatively affects the accuracy of my file. Should I talk to the same doctor? Should I ask for it to be changed? Is it even possible?
