Is a rectal exam standard during a female pelvic exam at the gyno?

Female 33, 22 at the time, Caucasian, NE USA, one prior terminated pregnancy, no prior medical issues.
This is a rather general question:
I had a doctor when I was much younger thrust one or more fingers into me without any warning and then when I reacted in pain he asked coldly, “you haven’t had a rectal exam before?” I hadn’t, and not again in the ten years since. Is this concerning or just bad bedside manner? TIA.
