rx:
60mg Vyvanse upon waking
300mg Quetiapine Fumarate XR with dinner
75mg Quetiapine Fumarate IR at bed time
10mg Olanzapine PRN (very rarely needed)
3 months I had to get my scrips sent to another pharmacy, as work had required me to be in another city. I noticed that my 300mg was suddenly hitting me very hard, and I was impaired at approximately 7pm, often falling asleep on the couch, but then being alert again at 9pm, taking my IR, and then going back to bed around 11pm.
I was wondering why this happened, and I noticed that my pills were different, so I returned to the pharmacy with the bottle, and asked them if there was any know difference in effect between generic manufacturers of the drug. The pharmacist told me that this is not reported with Quetiapine, and that the only antipsychotic that is known to have that issue is Lithium. She suggested I keep up with it for a while, and that I'll likely soon notice that it was in my head. Okay, that's fair enough.
So today I ran into a problem with the doctor's office not wanting to refill my prescription via telephone (I saw a doctor other than my family last time I got them refilled. Because of this, they weren't willing to ask my family doctor to renew them, and the other doctor was on 3 months leave), so I went to a prescribing pharmacist to get me through until I could see my doctor. This is where things got goofy. She goes "UH, you're not on extended release" and I say, "Yes I am?!" she goes into the back area, comes out with the bottle of Quetiapine, and shows me that for the last 3 months I've been taking 300IR with dinner, and 75IR after dinner. She said "I need to make some calls. Give me two hours please." So I do, then I come back a bit later and she explains that the original pharmacy had screwed up when they forwarded the script, and that YES my actual prescription is for XR, but I had been taken IR. She asked me what I wanted to do, keep up with the IR until I can see my doctor, or switch back to XR.
I elected to switch back the the XR, but now I'm terrified that my tolerances are all screwed up, and I could be flung into an episode. As this was the same dosage over the same amount of time (though in different formulations) am I likely to be messed up?? This explains a ton of problems I've been having over the past few months, and I'm glad to have gotten it sorted, but I am very worried this will be difficult to recover from. I'm hoping somebody can offer some encouraging words, or advice.
