4M stomach bug high heart rate with fever and vomiting

My son, 4 years old, has been sick with a fever, cough, and vomiting for 5 days. He vomited 4 times during days 1-3. He's had a mix of 7.5ml of acetaminophen or ibuprofen back and forth 3 times a day for 5 days. He's had children's cough syrup (Dextromethorphan HBr USP 20 mg / 20 ml, Guaifenesin USP 200 mg / 20 ml) 2 times a day for the last 3 days. Half a packet of Emergen-C yesterday eveningnaround 9 pm (it's 7 am here). He had acetaminophen and cough syrup last night at 6pm, and then 1mg of melatonin around 9pm . Currently, his heart rate is between 135-145 bpm, his Oxygen saturation is between 94-96%, and his temp is 101.3°F. Is this a normal heart rate for this situation? At what point of continued symptoms should he go to the Dr./Urgent care?
