Question regarding tore extensor tendon hood and sliding tendons.

PROBLEM:

I have this odd thing going on with the tendons of my hands right now.

I have had years of hand/wrist/finger pain where cause hasn't been found (carpel tunnel ruled out), but I flicked a fly off my chest in May, somehow tore extensor tendon hood in middle finger (aka Boxers Knuckle, doctors initially didn't believe it was caused by such a small action), so they sent me to plastic surgeon for repair.

Of course had to wait to see if would heal on its own first so sit for a couple months in brace, it didn't heal. But during this time we notice that all of my tendons in both hands are sliding off my knuckles, which shouldn't be happening.

The plastic surgeons best guess was "maybe you have worn out your hands" but wanted me to see a rheumatologist for any potential problems.

After LOTS of blood work and even a bone density scan, we only find some mild osteopenia but nothing auto-immune related.

Now it is November, still dealing with swelling where that hood ruptured (amount of swelling dependent on use), tendons sliding off knuckles, and pain in both hands.

THEORY:

Been a tech early adopter since I was a kid, have spent a lot of years typing. For the last 20 years about 15 hours a day.

In my own research came across a condition called "tendon snapping" where fibers holding the tendons can "wear out" causing the tendons slip out of place over the edge of joints.

My theory is that this has happened in my hands resulting in the tendons snapping out of place but it happened gradually and unnoticed.

I wonder if some of the pain in my hands was caused by the tendons pushing against the extensor tendon hoods resulting in them being weakened from this but in May when I flicked the fly it caused the tendon to move too rapidly and finally caused the that extensor tendon hood to fully rupture.

  1. Could this be feasible?
  2. Should I be concerned about extensor tendon hoods in other fingers?

POSSIBLE SOLUTION:

It looks like standard treatment for "tendon snapping" when it does not heal on its own is surgical and to use tissue that is already in place to rebuild the fibers that hold the tendons in place. Keep the hand in brace for healing, then physical therapy.

  1. Does this seem viable?
  2. Better to do each finger individually, or whole hand?
  3. Do both hands at once, or one hand at a time?
