GI DOCS - What to do if hemorrhoids are blocking the exit AND constipated?

I (26F) never struggle with the urge to go. I always know when I have to pass a BM, but there have been a number of times - including this very moment- where even when I want and need to pass a particularly large, hard, & constipated stool, I can’t bc of all the hemorrhoids in the way & how painful they get when on the toilet.

It feels like some are blueberry sized and some are skin tag like. They also protrude over each other so it’s hard to give an exact number of how many hemorrhoids I’m working with.

At the moment I am struggling with a constipation episode and have tried a ton of Miralax & that Calm magnesium citrate powder. I’ve also attempted using a Fleet enema but only like 2 mLs made it before leaking out, and it didn’t seem to soften the stool enough to exit.

If it were up to me I’d poop everything out right now but sitting and attempting to evacuate is so painful, and nothing can get past the external hemorrhoids.

Is there something other than a hemorrhoidectomy manual disimpaction that could help?
