Memory Leak? If that's what it is?

Hello all. I hope I can get some help for my issue here.

I have a 1 1/2 year old desktop Lenovo. I'll post the info below. On the regular, weird things happen. Browser windows will just randomly close. A game I am loading will take forever to load up, and may just hang. I'll get notifications about being out of memory, or a website might send me a warning notice about there's been a spike in video processing usage, or something like that, and the page can't show what it's meant to show. Things slow down, or won't load, and today after rebooting, I got an odd error message that I've never gotten before. I'm no programmer or hardware pro, but I'm not completely clueless, but it feels like something it gobbling up chunks of memory, leaving other legitimate stuff to hang, crash or just close. Here are my stats and the error message from today's reboot:
