Flat, overly greasy cookies? What the heck did I do wrong?

I used this recipe which honestly comes out different every time. But this time it came out terribly. I don’t know what I did wrong. The top layer of the cookie flakes apart from the rest of the cookie which looks really greasy. Maybe my baking soda is too old? Not enough flour? Didn’t refrigerate long enough before baking?
