Nothing new in tank for nearly two years but one fish looks like it has ich

I was under the impression that once ich was truly eliminated from an aquarium it's gone for good unless you get contamination from an outside source. I had a bad case of ich a couple years ago and haven't had any issues since then. I haven't added anything to the tank since then.
As far as I can tell, none of the other fish have anything that looks like ich on them. Is there another disease that looks like this?
