Need help i don't know what could have caused this

i have just finished my first helmet and i have no idea how it got messed up in the process i'm at a lost as to what to do to avoid this problem in the future. And i just realized that cura 5.0 cut the top of the helmet off that's weird. still new and learning i checked the belt and the wheels and everything is tight like it should be Please Help.
